What does the program involve?

This program is being offered across the county. It will help you take steps toward positive change and healthier living as you build your confidence and your ability to manage your day-to-day life.

  • Medication “how to’s”
  • Personalizing a fitness and exercise program
  • Relaxation techniques
  • Dealing with negative emotions
  • Managing symptoms
  • Improving communication
  • Tips for eating well
  • Setting goals
  • Effective problem solving

Who Should Attend?

These days, more and more of us are living longer with chronic health conditions. Anyone can attend, regardless of age, with a long-term health condition such as:

  • Heart Disease
  • Asthma
  • Lung Disease
  • Diabetes
  • Emphysema
  • Parkinson’s
  • Other Chronic Conditions
